COVID-19 Vaccines
Family Medical Center offers Covid vaccines (1st dose, 2nd dose and boosters) for all ages (6 months and over). Vaccines are by appointment only. Call for appointment 517-263-1800 – Lenawee County and 734-654-2169 Monroe County.

COVID-19 Vaccine Distribution For Family Medical Center Of Michigan Patients
Reserve your spot by completing the COVID-19 Vaccine “Save Your Spot” Form
When additional doses become available due to canceled appointments or no-shows, the staff have a process of contacting people per State guidelines to ensure the doses are administered to people who are currently eligible to receive the vaccine. You can also complete the COVID-19 Vaccine “Save Your Spot” Form
Please note:
You should not receive the second dose earlier than recommended (21 days for Pfizer and 28 days for Moderna). The second dose should be given as close to the recommended timeframe as possible, but if that is not possible, then the second dose of Pfizer and Moderna COVID-19 vaccines may be scheduled up to 6 weeks (42 days) after the first dose. However, there is no clear data on the effectiveness if it is delayed beyond this timeframe. For more information, review the CDC’s guidance on vaccine administration.
