If you need help losing weight, eating healthier, managing your medications, monitoring your blood pressure, or monitoring your blood sugar, Family Medical Center can help!
Family Medical Center will provide you with a Care Team to work with you and for you. Members of your team will serve as an extra voice between you and your provider and be an extra layer of support for you.
Your Care Team, which consists of your Primary Care Provider, Care Managers, and Community Health Workers, will help you create a plan to manage your health and help you stay on track with your plan to meet your goals!
Our Community Health Workers and Care Managers also help connect you with other community resources you may need like transportation to and from your appointments, food, or financial assistance.

Eligibility Requirements
Medicare CCM
If you have TWO or more chronic health conditions such as:
- Arthritis
- Diabetes
- High blood pressure
- Depression
- COPD
- Vascular Disease
Blue Cross Blue Shield (PDCM)
There are no eligibility criteria for Provider Delivered Care Management (PDCM) through Blue Cross Blue Shield
