The Pam LaPan Concierge Program helps eligible Medicare patients connect with services, resources, and additional support that can make it easier to stay healthy and get the care they need.

The Pam LaPan Concierge Program is another way Family Medical Center works to make health care more connected, accessible, and responsive to the needs of our patients.
There is no cost to participate in the Pam LaPan Concierge Program.
Helping Remove Barriers to Better Health
Sometimes staying healthy can be affected by challenges outside the doctor’s office.
If you are having difficulty with food, transportation, housing, medications, behavioral health, health care costs, or another concern, our Concierge Program team can help you explore available options.
When a need is identified, we can connect you with Family Medical Center programs as well as community organizations and resources that may be able to help.
How the Concierge Program Can Help
Depending on your needs and eligibility, the Pam LaPan Concierge Program can help connect you with:
- Free Annual Adult Wellness Visits to review your health history, identify care needs, and create a personalized wellness plan
- Prescription delivery in Lenawee and Monroe Counties
- Medication savings programs, including 340B and other available discounts
- Medication counseling
- Medication packaging and synchronization to make managing prescriptions easier
- Immunizations
- Food assistance and resources
- Transportation support
- Housing support
- Behavioral health services, including walk-in consultation at our Temperance location during business hours
- Assistance with Family Medical Center’s Sliding Fee Discount Program for eligible patients
- Care management or remote patient monitoring, when appropriate

How the Program Works
Eligible patients who receive their Medicare primary care through Family Medical Center may be automatically enrolled in the Pam LaPan Concierge Program.
Our care team may reach out periodically to see how we can support you. You can also talk with your Family Medical Center provider during a visit and ask about a referral to the Concierge Program.
There is no cost to participate in the Pam LaPan Concierge Program.
Some additional services, such as care management or remote patient monitoring, may have a copay depending on your insurance coverage. If a service may involve a cost, your care team will explain it before you decide to participate.
