- Have you had your well visit yet this year? If not, you may hear from us!
- Have you missed any preventive services you are due for? If so, you may hear from us!
- Have you recently been in the hospital or Emergency Room? If the answer is yes, you may hear from us!
Our certified Community Health Worker staff can help you arrange appointments and assist you in making sure you keep them by helping address any barriers you may have. If you need additional on-going assistance, they may even refer you to care management services!
